Group Policy not applying even though user is in security group

I setup a group policy with user settings and set security filter to only apply settings to specific group.  I also added authenticated users and granted read access in delegation tab.  I added a user to the security group 5 days ago and today they told me that they still did not have the setting that the group policy should have implemented.  I open command prompt as admin and ran gpupdate /force /target:user and after it completed I had user log off then on and policy still did not apply, I ran command again and then had user reboot still no luck.  I ran gpresult /v and listed that the policy was not applied "Filtering:  Denied (Security)", and also in the section below where it list the groups the user is a member of, the group for that GPO was not listed.  I double checked to see if user was still a member of the group in AD Users & Computers and she was, I even checked different domain controllers to confirm it wasn't some AD replication issue.  User works outside the office, I confirmed that she was connected to VPN.  Not sure why the policy is not applying and do not know what the next step is.  Any help would be appreciated.
